Monroe County Treatment Resources for Teens and Families

Melissa Hayward from Monroe County County OMH offers resources for supporting schools’ capacities to link students and families with county-based supports through Medicaid and Non-Medicaid resouces.  Ms. Hayward discusses resources including SPOA, FACT, Health Homes, Child and Family Treatment and Support Services, and others.

Family Access and Connection Team (FACT)

FACT is Monroe County’s integrated entry point to care for our community’s highest needs/ highest risk children and youth, incorporating the Monroe County Office of Mental Health, Office of Probation – Community Corrections and Department of Human Services.

Child & Family Treatment Support Services (CFTSS)

Allows interventions to be delivered in home and other natural community based settings where children and their families live.  Offers lower intensity services to prevent the need for more restrictive settings and higher intensity services. Avail. for youth with Medicaid.

Children & Youth Single Point of Access (SPOA)

SPOA is a centralized access point for youth in Monroe County to connect to enhanced Mental Health and Behavioral Health Supports. SPOA is for children and youth with mental health concerns whose needs extend beyond outpatient therapy and/or medication management

Children's Health Homes of Upstate New York (CHHUNY)

A Children’s Health Home is a type of care that allows for health care and service providers in the community to work together to make sure that your child receives the care and support services to stay healthy.   This is done by a care manager who oversees and links to services.

Behavioral Health Crisis Call Line

Available 24 hours a day, seven days a week for people coping with mental health issues.  The URMC Crisis Call Line is staffed by master’s level counselors trained in crisis management. The counselor will listen to you, help you talk through the problem and suggest possible solutions.  Call 275-8686.

UR Med Outpatient Services - Pediatric Behavioral Health & Wellness

PBH&W Outpatient Services meet the needs of families from diverse backgrounds who may benefit from outpatient assessment and treatment. 

Monroe County Walk-In Clinics

List of services where walk-in times are available and a list of providers with basic contact information to use to inquire about appointment availability. Please call the center for current walk-in hours before traveling to their location. 

UR Med Child & Adolescent Partial Hospitalization

CAPH serves as an alternative to psychiatric inpatient admissions or re-hospitalization for adolescents. The program includes individual, group and family therapy, psychiatric evaluation and evaluation to determine if medication therapy could be appropriate.

UR Med Child & Adolescent Inpatient Hospitalization

The inpatient program serves patients ages five through 18 who have symptoms of acute psychiatric illness or disorder requiring a safe, therapeutic, and supportive environment to assist with stabilization interventions. Admissions to the service generally occur on an emergency basis through the Emergency Department’s CPEP.

Monroe County Mobile Crisis Team

MCT can be accessed by dilaing 2-1-1.  MCT is a multidisciplinary team of mental health professionals will visit you or your family in your home, place of employment, school or any other location in Monroe County. Services are designed to help divert individuals from the county’s psychiatric emergency departments and prevent unnecessary emergency room visits, when possible.
